This Waiver, Release, and Hold Harmless Agreement is made by and between the undersigned and
ART OF PILATES.
- Attendance at ART OF PILATES and use of its equipment and facilities are at the participant’s own risk.
- ART OF PILATES is not liable for any injury, damage, or loss sustained on or around the premises.
- The participant assumes full responsibility for any injuries that occur while using ART OF PILATES facilities.
- The participant fully releases ART OF PILATES, its owners, employees, and agents from all claims, demands, and causes of action.
- The participant confirms they are in good physical condition and have no condition preventing safe exercise.
- The participant agrees to notify ART OF PILATES if their health condition changes or if pregnancy occurs, and provide medical clearance if required.
- ART OF PILATES is not responsible for loss, theft, or damage to personal belongings.
- All studio rules and safety regulations must be followed.
